CLARIFICATION TEXT FOR THE PROCESSING OF PERSONAL DATA

This Clarification Text has been prepared to inform data subjects about the procedures and principles regarding the processing of personal data collected through the closed-circuit recording systems used in the building and annexes of YAŞAM SAĞLIK HİZMETLERİ VE TIBBİ ARAÇLAR TİC. ve SAN. LTD. ŞTİ. (hereinafter referred to as the “Company”) in accordance with Law No. 6698 on the Protection of Personal Data (the “Law”).

2. PERSONAL DATA TO BE PROCESSED

This includes the images of individuals captured within the camera's field of view and vehicle license plate data of those entering the Company’s building, warehouse, and parking areas.

3. PURPOSE OF PROCESSING PERSONAL DATA

The personal data collected are processed within the scope of the personal data processing conditions and purposes specified in Article 5 of the Law, with the aim of ensuring the legal, technical, and commercial security of relevant persons with whom the company has a business relationship, safeguarding the security of the Company’s buildings and annexes, protecting the safety of employees, customers, suppliers, and Company property, and detecting potential unlawful acts or obtaining evidence thereof. Cameras operate 24/7.

4. METHOD AND LEGAL BASIS FOR COLLECTING PERSONAL DATA

Personal data are collected via camera systems installed in the Company’s buildings and annexes as part of the Company’s security policy. The data are processed based on the legitimate interests of the data controller in accordance with Articles 5 and 6 of the Law.

5. PARTIES TO WHOM PERSONAL DATA IS TRANSFERRED AND PURPOSE OF TRANSFER

Your personal data may be shared with authorized public institutions and organizations in accordance with the above-mentioned legal regulations and purposes, ensuring administrative and technical security measures are in place.

6. RETENTION PERIOD

Camera recordings are retained by the Company for one month. When the purpose requiring their processing ceases or when the retention period specified by legislation expires, they are destroyed.

7. RIGHTS OF DATA SUBJECTS

As the owner of personal data, you have the following rights under Article 11 of the Law:
a. To learn whether your personal data is being processed,
b. To request information if your personal data has been processed,
c. To learn the purpose of processing and whether the data are being used in accordance with this purpose,
d. To know the third parties to whom your personal data are transferred domestically or abroad,
e. To request correction if your personal data are processed incompletely or inaccurately,
f. To request deletion or destruction of your personal data,
g. To request notification of the actions taken pursuant to items (e) and (f) to third parties to whom your data have been transferred,
h. To object to the emergence of any adverse results through analysis of your personal data exclusively by automated systems,
i. To request compensation if you suffer damage due to unlawful processing of your personal data.

In compliance with Article 4 of the Law, it is essential that the personal data we collect and process be accurate and, where necessary, up-to-date. If there are any changes to your personal data, you may update it through the methods outlined below.

8. METHOD OF APPLICATION TO THE DATA CONTROLLER

In Writing:

Submit a signed copy of the “KVKK Application Form” along with a document verifying your identity to our address ESATPAŞA MAH. ABDİ İPEKÇİ CAD. NO:7 MENEMEN, İZMİR either in person, through an authorized representative with a notarized power of attorney, or via a notary.

Electronically:

Sign the “KVKK Application Form” using a “secure electronic signature” defined by the Law on Electronic Signature No. 5070 and send it to the Company’s Registered Electronic Mail (KEP) address or the e-mail address yasam@yasamsagliktipmerkezi.com using the e-mail address you previously registered with the Company.

For identity verification purposes, additional methods (such as contacting your registered e-mail address or phone number) may be employed. If the application is submitted by a third party on behalf of the data subject, a notarized special power of attorney must be provided.

The Company will respond to your requests, depending on their nature, as soon as possible and no later than 30 days, either in writing or electronically.
